When a Cat’s Back Legs Suddenly Stop Working

Feline aortic thromboembolism — more commonly known as a saddle thrombus — is one of the most frightening emergencies a cat owner can face. One moment your cat seems normal; the next, it cannot use its back legs and is crying out in pain. A blood clot has lodged at the base of the main artery (the aorta), cutting off blood flow to the hind limbs.

A new multicenter study published in Frontiers in Veterinary Science looked at whether a clot-dissolving drug called tissue plasminogen activator (tPA) — the same family of drugs used to treat human strokes — could improve outcomes in 183 cats with this condition. The findings offer an honest, nuanced picture: the drug was linked to better blood flow and limb recovery, but it did not help more cats survive to leave the hospital. Sudden death was also more common in cats that received the drug.

What Is a Saddle Thrombus — and Why Is It So Serious?

A saddle thrombus gets its name from the shape of the clot: it straddles the point where the aorta splits into two arteries supplying the hind legs, like a saddle across a horse’s back. The clot blocks blood from reaching the back legs, causing sudden, severe pain, paralysis or weakness in the hind limbs, and cold, pale, or bluish paws.

The underlying cause is almost always heart disease — usually a condition called hypertrophic cardiomyopathy (HCM), where the heart’s muscle walls thicken and blood can pool in abnormal ways, forming clots. Because the blockage is so sudden and severe, saddle thrombus is a true emergency with a poor prognosis even with the best care.

The sobering finding: Despite that improved blood flow, survival to discharge was similar between treated and untreated cats (34% vs 35%, p > 0.999). Sudden death was more common in cats that received tPA (46% vs 19%, p = 0.002).

The authors raise reperfusion injury as one possible explanation for sudden deaths. When blood returns to oxygen-starved tissue, harmful byproducts can enter the bloodstream and may trigger complications including hyperkalaemia (a dangerous rise in potassium), which can provoke life-threatening arrhythmias. In this study, however, measured reperfusion-related complications did not differ significantly between groups: acute kidney injury occurred in 10% (tPA) versus 3% (no tPA) (p = 0.237), and hyperkalaemia occurred in 14% versus 8% (p = 0.359).

What This Means if Your Cat Has Heart Disease

Saddle thrombus is most common in cats with underlying heart disease, especially HCM. If your cat has been diagnosed with a cardiac condition, this research highlights why close monitoring and knowing the warning signs matters.

Act immediately if you see sudden hind-leg paralysis. A cat that suddenly cannot use its back legs, is crying in pain, or has cold or pale paws needs emergency veterinary care within minutes — not hours.

Treatment decisions are deeply individual. Whether tPA is the right choice for a specific cat depends on how long ago the clot formed, the cat’s overall cardiac condition, and the treating team’s assessment. Only the veterinary team evaluating your cat in real time can weigh those factors. This study shows the drug can restore blood flow but carries a real risk of sudden death.

Blood-thinning medication may reduce clot risk. Cats with HCM are sometimes prescribed medications like clopidogrel to lower the chance of clot formation in the first place. Ask your vet whether preventive treatment is appropriate for your cat’s cardiac diagnosis.

The Limits of This Research

This study is valuable, but it has important limitations worth understanding.

Because it looked back at existing records, the researchers could not control which cats received tPA and which did not. Cats selected for tPA treatment may have differed in ways that affected their outcomes — for example, they may have been in better overall condition or been seen earlier after the clot formed. This kind of selection bias is an inherent challenge in retrospective studies.
